Transition guide

Last updated:

Moving from childhood to adulthood involves change and decision making for all children and their families; this process is likely to involve different and additional considerations when the young person has a Learning Disability and/or Autism.

The transition pages on our website are designed to pass on useful information to young people aged around 14 to 25, and their families or other supporters.

They do not take the place of existing publications, for example North Yorkshire Preparation for Adulthood and the Down Syndrome Association Transition to Adulthood, however, the information in our transition guide is tailored to the Harrogate and surrounding area and will be kept as up to date as possible. Please contact us with any inaccuracies or updates that you might spot; the pages will be updated monthly.

Everyone is welcome to access this information and/or to contact Ruth McQuire for any further information (Transitions and Outcomes Manager ruth@hs4lc.org.uk ). We have a meeting space at the Harrogate Chocolate Factory Café which is ideal for getting to know people so if you would find it easier to access any of the information by sitting down and talking to someone, please get in touch.
